Salary: £26,000 – 28,000 per annum pro rata / 18 hours per week
Living with a Serious Mental Illness (SMI) can reduce life expectancy by 10-20 years. Join this local charity on their mission to empower and support those living with a diagnosis of SMI to become more independent and able to look after their health.
This role has been created by a new pilot service within the South West London Local Mind Partnership. The collaboration enables the transformation of outcomes for those with SMI.
You’ll work with Primary Care Networks, GP practices, health clinics and existing local SMI providers to successfully target and engage the hardest to reach individuals and remove barriers that people living with SMI encounter when accessing their annual health checks. .
You will need great planning, organisational, communication and interpersonal skills as you work flexibly and proactively to coordinate activity across the borough of Richmond. Managing a wide range of relationships and partnerships, as well as your team, is a key part of this role.
